The IV & Wellness Institute aims to improve your quality of life. It is our mission to provide you with the best treatment in a safe and comfortable manor. In order to achieve this, we have a series of protocols to identify high risk patients who may require a higher level of medical care. To ensure maximum patient safety, some may not be eligible to receive certain treatments. We thank you for your understanding.
You certify that you understand and agree with the following statements:
- I have read, understand and agree to the Medical Disclaimer and Terms and conditions of The IV & Wellness Institute.
- All the information in the medical screening is complete to the best of my knowledge and is accurate.
- I understand that whilst I am receiving treatment, I will not be allowed to leave the supervision of The IV & Wellness Institute staff/direct premises monitored by Infinite staff.
- If I require the restroom during an infusion, I accept to notify The IV & Wellness Institute staff and my IV may be removed andsubsequently replaced at my request.
- I understand the risk of IV catheter insertion includes pain, bleeding, bruising, blood clots, infections andlocal soft tissue injury.
- I understand to seek immediate medical attention if such complications arise or if requested by The IV & Wellness Institute staff.
- I understand that all people react differently to treatments and whilst most will have the desired resultswith The IV & Wellness Institute, there is no guarantee that I will feel the same way or see the exact expectedresults.
- I understand that The IV & Wellness Institute staff have the right to refuse and terminate my infusion at any point and that Imust comply with the instructions given to me for the duration of the treatment.
- I certify that I am not intoxicated or under the influence of any mind-alternating substances at the time ofthis elective therapy
- The IV & Wellness Institute does not accept any responsibility or liability for any harm, loss or death that mayoccur in any form including as a result of an allergic reaction to the medication, undiagnosed or underlying medical conditions, undisclosed medication, medical conditions or any breach of the above mentioned medical disclaimer and term and conditions.
